Signature, print name, date: what goes where
Nearly every form ends with the same three lines. They look simple, yet forms get sent back because a name was written in the signature line or the date was missing. Here's how to fill each one in — right on the PDF.

Avoid these
Signing on the name line
The print-name line is for readable letters, not your signature.
Nicknames
Print the name the form or contract uses — usually your legal name.
Ambiguous dates
04/05 can be April 5 or 4 May. Match the form or write the month as a word.
Filling in the three lines
Zoom in so everything lands neatly on its line.

Why forms ask for a printed name
Signatures are often hard to read. The printed name tells the reader exactly who signed. Use block capitals if the form asks for them.
Title and company lines
Some forms add “Title” or “Company”. Fill them in if you sign for an organization; leave them blank if you sign for yourself.
Doing it on a PDF
